Financial Guidelines
We have always maintained that open communication is the best policy, the following financial guidelines are presented. If you have any questions or a particular situation you would like to discuss, please contact the office administrator.
1) There is usually an initial fee for our services and a lesser fee for follow-up visits.
2) Surgical fees include all follow-up visits (not including x-rays).
3) Dr. Green participates with most major insurance plans as well as workers compensation claims and Medicare.
4) If you have health insurance, we will be glad to assist you with your insurance reimbursement. Any payment not received by your insurance company within 60 days becomes the patient's/guarantor's responsibility. Your insurance plan may only cover a portion of your medical costs, therefore you will be responsible for any outstanding balances.
5) Co-pays are due at the time of each visit.
6) In order to maximize your insurance benefits if a surgery is planned, we will prior-authorize your surgery for you (if required). We will also contact your insurance company to confirm benefits.
7) If you have any outstanding balances after your insurance company has paid their portion, this amount is due as soon as possible. If you find that you need to make monthly payment arrangements, the office administrator will gladly sit down with you to work out a payment plan.
